40 year old male patient with c/o fever since 2 days
40 year old male patient came to the op with
*Chief complaint of
*Fever since 2 days with generalised weakness
* Throat pain and difficulty in swallowing
History of present illness: -
Patient was apparently asymptomatic 1 week ago after which he developed fever with generalised weakness and throat pain and difficulty in swallowing later he came to the hospital for treatment .
Alcohol dependence syndrome
Tobacco dependence syndrome
Constipation
History of past illness:
No similar complaint's in the past
Not a k/c/o DM ,HTN ,TB, epilepsy ,asthama
Personal history:-
Diet : mixed
Appetite : normal
Bowel and bladder : regular
Sleep : inadequate since 1 week
Addiction: alcohol and ciggerate
Any drug allergy : not known
Family history:
Mother with diabetes
General examination : -
Patient is concious , coherent and coperative . Well oriented to time , place And person.

Cyanosis : absent
Clubbing : absent
Icterus : absent

Temperature : 100 °F
BP : 100 / 70 mmHg
Pulse Rate :125 bpm
Sp O2 : 97%
