45 year old female came to OP witt
CHIEF COMPLAINT OF
Fever since 10 days

HISTORY OF PRESENT ILLNESS

Patient apparently asymptomatic  10 days back then she developed high grade fever with Multiple ulsers in the ora cavity, burnig sensation of tongu,pain during swalloing ,dry buccal mucosa

HISTORY OF PAST ILLNESS
NO similar complaint in past

Married
Occupation-labour
Appetite-normal
Bowels-normal
Micturation- normal
NO known relevant drug allergy
No addictions

MENSTRUAL HISTORY

Age menarche-13years 
Duration of cycle 5/30(past)

GENERAL EXAMINATION

Patient is concious coherent and coperative Well oriented to time place and person
No Pallor
No Clubbing of fingers
No Cyanosis
No Dehydration

SYSTEMIC EXAMINATION
